The difference lies in the approach. The answer is careful selection: outcomes approach 90% in patients whose symptoms have persisted and who have already tried conservative and pharmaceutical treatment without success. <strong>Patient selection, in other words, is the single most important factor.<\/strong><\/p><p><span style=\"color: #1e4080; font-size: 18px;\"><strong>Aggressive versus limited discectomy<\/strong><\/span><\/p><p>An important technical question is <strong>how much disc is removed.<\/strong> Some surgeons remove the whole disc (<strong>aggressive, or subtotal, discectomy<\/strong>); others remove only the fragment causing the compression (<strong>limited discectomy, or sequestrectomy<\/strong>).<\/p><p><strong>The aggressive approach<\/strong> reduces the likelihood of recurrent herniation. Its disadvantage is loss of disc height at the operated level: unless instrumentation is placed to support the segment, it is common for the vertebrae to settle towards one another and for instability to develop, typically producing low back pain years later.<\/p><p><strong>The limited approach<\/strong> carries a higher recurrence rate but avoids that risk. In theory one could place instrumentation in every patient and perform an aggressive discectomy \u2014 but instrumentation makes the operation considerably more complex, with higher complications and harder recovery, and is therefore rarely the right answer.<\/p><p><strong>In practice, limited removal has become the standard<\/strong>, accepting a small risk of recurrence. If a recurrence does occur, it does not necessarily press on the nerve again: the operation itself widens the canal, so there is more room available than there was before.<\/p><p><span style=\"color: #1e4080; font-size: 18px;\"><strong>The procedure<\/strong><\/span><\/p><p>The operation is performed either through a very small incision of 1\u20132 cm (minimally invasive microdiscectomy) or through a larger incision of 3\u20134 cm (conventional discectomy). The bone and ligament obstructing access are removed so that the disc fragment can be taken out safely, and so that additional space is created behind the nerves \u2014 which is why, as noted, a recurrence at the same level may not produce significant compression.<\/p><p>The published evidence shows no difference in final efficacy between the two. What microdiscectomy demonstrably offers is a lower complication rate, a shorter recovery, and a smaller, less visible scar.<\/p><p><a
